My name is Nikki Crites. I am a Community Health Worker for the Community to Home
Program, in Pennsylvania. The Community to Home program is designed to improve
health outcomes to those, Children and Youth with Special Health Care Needs, who are
impacted by health disparities. To be eligible for this program, the individual will meet
the outlined criteria.
Children 21 years of age and under.
Newly diagnosed children with special health care needs or newly residing in a
PA rural county.
Households with an income of 300% or less of the federal poverty level.
Once an individual is accepted into the Community to Home program, they will be
assigned a Community Health Worker. The Community Health Worker will provide
assessment and care planning over a six-month period. Community Health Workers can
assist families in attendance to appointments, roadmaps to self-sufficiency, linkage with
community resources, and support and understanding as they navigate their child’s
needs.
